My office is located within the shared office space of Sage Center for Health and Wellness in Beaverton's Executive Park Plaza West, Building 3, Suite 560.
Office suite (Suite 560) is on the first floor of Building 3 when you enter.
The Park Plaza West is near Exit 2A off Hwy 217. Free and accessible parking available next to the buildings. The complex has Buildings 1, 2, and 3. I am located in the last building (Building 3) which is the farthest from the main road of Beaverton-Hillsdale Hwy.
Fees for Individual Therapy:
Current private pay rate: (as of May 1, 2026)
$170 for initial intake session (60 min)
$150 for 60 min therapy session

Original Medicare with all Medicare supplemental plans
Moda Health, including Connexus, Synergy, Affinity, OHSU PPO, OHSU EPO, HMC and OHSU Health, and Moda Select
Out-of-network provider:
Medicare Advantage plans operated by private insurance
Commercial insurance providers (except Moda)
I am able to provide a superbill/receipt, for possible reimbursement of services, if your plan has out-of-network benefits
